PMDC's Centralized Question Bank: The Most Overlooked MDCAT Reform
Every year after the MDCAT, thousands of students claim that the paper was "out of syllabus."
However, very few students know that the Pakistan Medical & Dental Council (PMDC) had already announced one of the biggest reforms in MDCAT history months before its implementation.
During PMDC's E-Kachehri held on 7 March 2025, the council announced a series of reforms aimed at improving fairness, transparency, and standardization across Pakistan.
Among these reforms was the following statement:
"A centralized Question Bank (Item-Bank) will be utilized to ensure inter-provincial uniformity."
Although this sentence appeared in PMDC's official communication, it received surprisingly little attention among students.
This single policy changes how MDCAT papers are prepared across Pakistan.

What Does PMDC's Centralized Question Bank Actually Mean?
The official statement is brief, but its practical implication is significant.
A centralized Question Bank means that PMDC prepares a unified repository of MCQs developed according to the national MDCAT syllabus. Provincial admitting universities—including UHS, SZABMU, KMU, SIBA, and BUMHS—draw questions from this centralized pool while preparing their respective provincial examinations.
The objective is straightforward:
- Maintain comparable difficulty levels across provinces.
- Reduce disparities between provincial examinations.
- Ensure that students compete on a more uniform assessment standard.
Instead of each province independently preparing entirely separate papers, all examination bodies now work from the same standardized question bank prepared under PMDC's framework.
Was This Policy Implemented?
Yes.
The centralized Question Bank mechanism was implemented during MDCAT 2025.
On 20 August 2025, Federal Minister for National Health Services Mustafa Kamal formally inaugurated PMDC's centralized Question Bank initiative.
Following the 2025 examination, many students reported encountering MCQs whose concepts matched textbooks used in provinces other than their own.
For many students, these questions appeared unfamiliar. However, under PMDC's centralized Question Bank policy, such questions were consistent with the national framework because the Question Bank incorporates content aligned with approved provincial textbooks.
This is one of the reasons many students believed the paper was "out of syllabus," whereas PMDC had already communicated its standardized Question Bank approach before implementation.

Why Many Students Never Heard About This Reform
Preparing students according to a centralized Question Bank is considerably more demanding than teaching from a single provincial textbook.
It requires:
- studying concepts across multiple provincial textbooks,
- identifying overlapping learning outcomes,
- creating a much larger MCQ repository,
- training teachers on multiple textbook variations, and
- continuously updating practice material according to PMDC policies.
Because of these additional academic and operational requirements, awareness of the centralized Question Bank has remained relatively limited among students.
As a result, many students continue preparing only from their provincial textbook without realizing that MDCAT assessment is now designed around a broader national framework.
Does This Mean Students Must Study Five Different Board Books?
Technically, the centralized Question Bank may include concepts represented across approved provincial textbooks.
However, for most students, independently studying five complete textbook sets is neither practical nor efficient, especially just weeks before MDCAT.
The better approach is to prepare from academic resources that have already been compiled according to PMDC's guidelines and the national syllabus.
